You can simply take a picture/scan it and send it to us by eBay messaging, email or text. No other documentation is necessary.

If you do NOT have a refrigeration license

You may download a PDF version of the required form here or you may fill out the form below my signature. You can then take a picture/scan it and send it to us by eBay messaging, email or text.
